Effective Tuesday, February 18, 2025, changes will be implemented in PowerSchool based on the 2025-26 Enrollment Form. Please share this information with staff at your school who manage enrollment in PowerSchool. Updated fillable enrollment forms are available on the Neighborhood Schools and Enrollment Options website under the Related Resources section.

The following areas of PowerSchool will be updated to reflect changes to the 2025-26 Enrollment Form:

  • Demographics page, under Students

  • Enrollment Form reports, under Print Reports/Print A Report

Changes to the Demographics page:

  • Question 28 – Revised to represent school climate survey for students in grades 7 – 12. The revised question refers to a general school climate survey, while the previous question was specific to the California Healthy Kids Survey (CHKS). Historical CHKS survey information can still be obtained by running the Enrollment Form - CHKS Opt Out report. Surveys are confidential and provide an "Opt Out" option for parents who do not wish their students to participate.

Changes to the Enrollment Form Reports:

To access the Enrollment Form report, select one or more students, choose Print Reports from the functions menu or select Print A Report when viewing an individual student. Then select one of the reports below. Please note that the 2024-25 reports will be removed at the end of the school year.

  • Enrollment Form-EN 2025-26 (English)

  • Enrollment Form-ES 2025-26 (Spanish)

New/Archived Enrollment Reports:

Enrollment Form - School Climate Opt Out This new report will list actively enrolled students whose families have opted out of the school climate survey. To run this report, select SqlReports from the Start Page left-side menu, then expand the group of Enrollment reports.

Missing or Invalid Demographic Data Report – This new report will check the student demographics page for missing, incomplete, or invalid student data, including parent/guardian and emergency contact information. Ensuring student records are current allows for accessible demographic information at all times, particularly in emergencies. Please use this report to identify and update any incomplete or invalid student data to ensure accuracy and compliance. To run the report, select sqlReports from the Start Page left-side menu, then expand the group of Enrollment reports.

Archived reports – The following reports will be archived and removed from SqlReports: Address Errors, Report and Progress Report Card Flags, and Missing Required Demographic Data. Please use the new Missing or Invalid Demographic Data Report to identify any missing, incomplete, or invalid student data.

Reminders:

When enrolling new students, be sure to enter the student’s full name and birthdate exactly as they appear on the student’s birth certificate or other legal documents.
