Effective Friday, November 1, 2024, the annual 2024-25 Facts for Parents booklet and the 2024-25 Universal Form will be available in the parent portal.
Facts for Parents is an annual publication containing updated information relevant to the families of San Diego Unified including rights and responsibilities as well as district, local, state, and federal policies that impact all students. Schools are required to provide the Facts for Parents booklet to all parents/guardians each year and to document that the parents/guardians have received it. The Parent Portal form provides schools with a way to track which parents/guardians have received Facts for Parents. For information on how to track Facts for Parents submissions, refer to the Tracking Facts for Parents & Universal Forms handbook.
Beginning this year, parents/guardians can complete or update the 2024-25 Universal Form online. This new form has been added to the other forms in the Parent Portal and allows parents/guardians to submit Photo/Video/Media Release information, Technology/Network Acceptable Use information, and acknowledge the Restorative Discipline Plan and Policy. The Tracking Facts for Parents & Universal Forms handbook is available to support schools with the collection process of this new form in PowerSchool. Note: If the Parent Portal is not used, school sites must manually enter this information in PowerSchool when a paper form is obtained from the parent/guardian. Under Forms, select the 2024-25 Universal Form to begin entry. Important: For schools that have begun submitting the universal form through the Annual Parent Authorization page, parents can review the information entered in the Parent Portal and finalize the 2024-25 Universal Form by simply submitting it. Alternatively, designated staff members can use the option in this form to indicate that the paper form has been submitted, or they can submit a help desk ticket to address any questions or concerns.
For families with parent portal accounts who need guidance completing the Facts for Parents or Universal Form online, school sites can direct them to the Parent Portal Resource Website in the Forms category of the Help tile to access the revised videos.

Photo/Video/Media Release Information – This report displays information from the Annual Parent Authorizations page Photography/Video/Media Release section. This report can be used to identify if parents have given permission to photograph/videotape students or credit students work, and the last school year this form was updated. To run this report, select sqlReports from the Start Page, then expand the group of Enrollment reports.

Technology/Network Acceptable Use Information – This report displays information from the Annual Parent Authorizations page Technology/Network Use Guidelines section. This report can be used to identify if students/parents have consented to technology/network use rules and conditions and providing access to the PowerSchool Student Portal, as well information on Internet access at home. To run this report, select sqlReports from the Start Page, then expand the group of Enrollment reports.

Uniform Discipline Plan – This report displays information from the Annual Parent Authorizations page Restorative Discipline Plan section. This report can be used to identify students who have read the Uniform Discipline Plan and understand the consequences if they violate the policy.
